Application Process

Are you a U.S. citizen or non-citizen national?
Have you ever been issued a U.S. passport before?
Were you at least 16 years old when your most recent passport was issued?
Was your most recent passport issued less than 15 years ago?
Is your most recent passport in your possession, undamaged and not reported lost or stolen?

You must apply in-person using Form DS-11

Based on your answers, you are required to apply for your passport in person at a passport acceptance facility (like ours!).

What you need to do:

  1. Fill out Form DS-11: You can complete it online and print it from the Travel.State.Gov website. Do not sign it yet!

  2. Gather your Evidence of Citizenship: You will need an original or certified copy of your birth certificate, naturalization certificate, or consular report of birth abroad.

  3. Bring Photo ID: A valid driver's license or state ID (along with a front-and-back photocopy).

  4. Get a Passport Photo: You need one 2x2-inch color passport photo. (Note: We offer photo services on-site!)

  5. Prepare your Payment: You will need two separate forms of payment: one check/money order made out to the U.S. Department of State and a credit/debit card for the acceptance facility execution fee.

You are eligible to renew!

Great news! Based on your answers, you are eligible to renew your passport.

What you need to do:

  1. Fill out Form DS-82: Fill out the renewal application on the Travel.State.Gov website, print it and sign it. Or, use their online form filler tool.

  2. Get a new Passport Photo: Staple one new 2x2-inch color passport photo to the application. (Note: We offer photo services on-site! Click here to book an appointment.)

  3. Gather your documents: You must include your most recent U.S. passport with your application. It will be mailed back to you separately.

  4. Calculate your fees: Include a personal check or money order payable to the "U.S. Department of State."

  5. Mail it in: Mail your documents (application, photo, payment method and your old passport) via USPS tracked mail to the address listed on the form. (Or, have us mail it for you!)

Passport Eligibility Information

Based on your answer, you are not currently eligible for a U.S. Passport. U.S. passports are issued only to U.S. citizens or non-citizen nationals.
